Care Coordination – Transitions of Care (Lafayette)
The Transitions Coordinator position exists to provide care coordination activities that support effective transitions of care across the health care continuum. In collaboration with Transitions of Care (TOC) Team members and community partners, the Transitions Coordinator is responsible for organizing, coordinating, and providing care coordination services across multiple healthcare settings to high risk patients.
Health Coach (Spanish Speaking – Boulder County)
This position exists to address social determinants of health and promote patient self-management skills through the provision of community resource referrals, education, motivational interviewing, goal setting, and follow-up with patients.
