STRIDE is a non-profit, Federally Qualified Health Center (FQHC) with a history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education and outreach and enrollment services in accordance with its mission. Patient Navigation Liaison
In order to provide a high-quality patient experience, the patient navigation liaison will work in collaboration with and complete tasks as a front desk, patient navigation and enrollment team specialist to assist patients through their visit in the STRIDE Convenient Care Clinics.
Essential Duties/Responsibilities:
- Demonstrates and facilitates a working relationship between patients and the clinic
- Provides excellent customer service to patients and employees
- Demonstrates professional behavior with the public, patients and peers at all times
- Demonstrates a high level of understanding of HIPAA and OSHA regulations
- Works closely and communicates consistently with internal and external staff to ensure continuity of care
- Assess for Social Determents of Health and refer to STRIDE care management services as needed
- Bridges the gap between clinician and patient by assessing needs and obstacles, following up with patients, and addressing health literacy, cultural issues, and social-class barriers
- Provides patients linkage to external, internal and community resources
- Develops and maintains lists of community resources as well as details on community programs
- Conduct chart review to acquire knowledge about patient status as well as past encounters with other STRIDE staff and programs
- Provides timely documentation and required reporting for all services provided
- Communicates and educates patients using native language, either in person or via interpretation services
- Performs general front desk duties as assigned including checking patients in, gathering information, assisting with patient calls, assisting patients with medical records and collecting copayments
- Completes daily batching of collections
- Assists patients in scheduling future appointments and linking to a primary care provider
- Maintains reception/lobby area supplies (i.e. patient information pamphlets, patient admission forms, etc.)
- Acts as liaison between patients and Billing Department regarding charges, payments, and insurance
- Answers and returns all calls in a timely manner; assesses callers’ needs and directs to appropriate personnel when required
- Accurately collects co-pays, deductibles and other out of pocket expenses at time of visit
- Promptly and accurately updates patient records including financial and demographic information
- Assess and assist patient for financial and eligibility needs and schedule appointment with the enrollment team when necessary
- Informs patients about the availability of government funded and discount programs
- Maintain knowledge and expertise in program eligibility, enrollment, and program specifications of Medicaid, Child Health Plan +, Medicare Savings, Title X, RWG, SDP, CICP, STRIDE Discount and Connect for Health Colorado
- Understand, interpret and communicate federal, state and eligibility criteria, policies and procedures
- Directs patients to Enrollment Specialist for financial intake and screening as needed
- Participates in community events to represent enrollment and navigation services
