Established in 2002, It Takes a Village, Inc. (ITAV) is an Aurora, CO based non-profit, tax-exempt organization geared towards reducing health and social disparities among people of color in the Denver metropolitan area.
The Medical Case Manager will provide client-centered medical case management, financial assistance, treatment education/support, linkage to care and support services for people living with HIV who are primarily coming out of incarceration.
Salary commensurate with experience, plus full health and dental benefits, sick, holiday and vacation leave.
Job Description:
a) Provides a range of client-centered services linking clients living with HIV to medical care,psychosocial and other services.
b) Helps clients create a coordinated care plan.
c) Provides ongoing assessment/reassessment of the client to determine further needs.
d) Monitors to assess the efficacy of the care plan.
e) Develops relationships with medical providers in order to ensure culturally responsive and appropriate care for clients.
e) Completes, in a timely manner, electronic and paper documentation, including financial assistance qualifying documentation, demographics, treatment adherence, medical lab results, etc.
f) Attends meetings and trainings as required.
Qualifications:
Bachelor’s degree, with emphasis on behavioral health or human services, preferred. At least two years of experience working with people living with HIV, people who are recently released from
incarceration, people with mental health disorders, or people with addiction disorders required. Must have extensive knowledge of the issues faced by communities of color and the LGBTQ+ community. Excellent computer and client file documentation skills required. Medical Case Management certification required within three months of hire. Motivational Interviewing training required within six months of hire.
