The Colorado Department of Public Health and Environment (CDPHE) has scheduled several dates in spring 2025 for the health navigator competency assessment. Passing this assessment places individuals on a state registry and allows them to be eligible for Medicaid reimbursement coming in July 2025.

To qualify for this assessment through the Patient Navigation and Community Health Worker Training Program, you must have completed the following courses:

  • Level 1: Health Navigation Fundamentals
  • Level 2: Care Coordination
  • Basic Motivational Interviewing
  • 3 eLearning course (Intro to Chronic Disease, Intro to Healthcare System, Preventive Healthcare 101)

To sign up for the assessment, you must first register on Certemy. CDPHE is utilizing this platform for the credentialing process. Once you register and your training is verified, you will receive a link to sign up for the assessment.

Please note, a new CHW assessment is in the process of being developed and availability for that assessment will be announced.
