Please Note: The Team-based Care Coordination ECHO series is a unique learning opportunity to connect online and practice care coordination skills. This series is NOT the same as the Care Coordination online course.

Care coordination is a set of shared strategies for preventing and managing chronic conditions to improve health, quality of care, and cost outcomes. This FREE six-week ECHO learning series is designed to help care coordinators, patient navigators, and community health workers better understand, communicate, and work at the top of their scope. Each session will introduce, build upon, and hone relevant skills for effective team-based care coordination for non-licensed staff working with patients.

Recommended Prerequisites: Level 1 Patient Navigator Fundamentals and Level 2 Care Coordination for Patient Navigators.

Commitment:
Six weekly* ECHO sessions held virtually
Tuesdays from 8- 9 a.m. (MT)
October 30th – December 11th, 2018

(*No session will be held November 20th in observance of Thanksgiving week)

This series is offered by ECHO Colorado in partnership with the Patient Navigator Training Collaborative (PNTC), the Rocky Mountain Public Health Training Center (RMPHTC) and the Care Coordination Central (CCC). Due to funding, Colorado residents receive priority for enrollment.
