The Community Outreach Specialist plays a critical role in improving patient outcomes by delivering outreach, engagement, and care coordination support focused on closing preventive and chronic disease care gaps.
The primary responsibility is to execute standardized outreach workflows at a high volume, while identifying barriers, documenting outcomes, and providing feedback to continuously improve outreach strategies. By working in collaboratively with participating health centers, the Community Outreach Specialist supports the implementation of outreach strategies, patient engagement efforts, and resource coordination that help improve performance in breast cancer screening, colorectal cancer screening, blood pressure control, and diabetes management while contributing to reduced avoidable utilization and enhanced patient experience.
While the position is primarily remote, occasional travel to participating health centers will be required to support workflow implementation, relationship development, and continuous quality improvement efforts across the network.

The Community Health Team Supervisor leads and supports a team dedicated to improving patient outcomes through culturally responsive outreach, preventive care, and chronic disease management. This role supervises community health staff, partners with community organizations and health centers, and helps patients access screenings, preventive services, and community resources.
The Supervisor also leads workforce development and implementation initiatives across the network, providing training, workflow design, data analysis, and strategic guidance to community health partners. By strengthening care coordination, population health efforts, and value-based care initiatives, this role helps improve quality measures, including cancer screening rates, blood pressure control, and diabetes management, while enhancing patient experience and reducing barriers to care.
