The American Diabetes Association’s (ADA) Standards of Medical Care in Diabetes highlight the importance of Community Health Workers (CHWs) in diabetes prevention and management, especially among underserved communities. Utilizing a team-based approach to care at the grassroots level by integrating CHWs as part of the health care team in diabetes prevention and management has been shown to be effective with cost savings. Participants will learn about the roles and skills CHWs need to be effective in their work.
