Designed for health navigators and community health workers, this course offers a hands-on journey into the heart of team-based care. You’ll build essential skills to boost client engagement, streamline workflows, and champion patient safety. Whether you’re new to care coordination or looking to sharpen your expertise, this course empowers you to become a driving force for high-quality, patient-centered care.
This course is instructor-led with one live learning session (LLS) per week. You will complete weekly homework before attending each LLS. The live sessions will be interactive with both large and small group activities. We ask that you participate in the live learning sessions by video to connect with your peers. The estimated time commitment is 3-4 hours per week.
Cost: $300
Sessions will be held on the following Thursdays, between April 30 and May 28, from 10:00 to 11:30 AM MT:
