Whether you’re new to care coordination or looking to sharpen your expertise, this course empowers you to become a driving force for high-quality, patient-centered care.
This course is 1.5 days in person and will be highly interactive with small group activities. No prework required. It is recommended to complete Level 1: Health Navigation Fundamentals before Level 2: Care Coordination.
