Goal of Meeting: Connect the PN CHW and PdS Community for the fall Alliance meeting.  This meeting will specifically highlight a couple of statewide resources to help patient navigators, community health workers and promotores de salud serve their communities.

Colorado Health Institute’s (CHI) Regional Health Connectors (RHCs) :Paige Backlund Jarquín (20 Minutes)

• Highlight key points from previous meeting (highlighting the 10 top issues we are hearing from the community and how we are addressing them)
• Invitation to join a working session to collaborate with Colorado Health Institute and the Regional Health Connector (this committee will be 60- minute session and held in November)
