Help your clients reach their goals! This online course provides tools and strategies to help clients around diet, exercise, diabetes and cardiovascular disease prevention and control.

This 4-week course includes both online coursework and live learning practice sessions. It is designed for patient navigators, care coordinators, clinical and non-clinical practice staff.

This course features 3 live learning session via Zoom:

  • Noon-1 pm MT May 3
  • Noon-1 pm MT May 10
  • Noon-1 pm MT May 17

Attendance at the live learning sessions and completion of 80 percent of course assignment is required to receive a certificate of completion.

You will receive information on accessing the online course when you register.

Prerequisite: Level 1 Patient Navigator Fundamentals Course (or see course instructor)

Additional Recommended Prerequisite: Advanced motivational interviewing (MI) or a total of 12 hours of MI training (or see course instructor)

Due to grant funding though June 2018, we are able to offer this course for a nominal fee to Colorado residents.
