Welcome to the Care Coordination for Patient Navigators Online Course!

Specifically, we will explore advanced care coordination and related concepts, including applications to practice and commonly encountered challenges. You will be guided step-by-step through the advanced strategies to enhance client engagement and team-based care for better health outcomes, patient safety, and workflow efficiency. While care coordination is still an emerging field, the science is catching up. In this course, we will be using research-backed tools for navigators that can help us help our patients become more capable and confident in taking actions towards a healthier life.



In this course, you will learn to:

  • Attribute all members of the care team with specific roles and functions in high-quality care coordination.
  • Communicate effectively with team members and patients to achieve the aims of care coordination.
  • Efficiently manage a patient panel with (and without) large-data management tools.
  • Use evidence-based strategies to support patient self-management goals.
  • Measure the effectiveness of team-based care coordination at the individual patient and population levels.

