The Patient Navigator (based in Denver, CO) will establish contact with cancer patients, survivors and caregivers through approaches agreed upon within the host hospital site, to raise awareness about information resources, support programs and services, and assisting in meeting other cancer-related needs of patients and caregivers. The Patient Navigator will guide patients and their families through the cancer care continuum to identify barriers to treatment and assist with access to needed resources to decrease barriers.

Bachelor’s degree required, preferably in health care related field or 2 years equivalent experience in oncology, social work, nursing, community health education or related field preferred.

Summer PN CHW PdS Alliance meeting

This meeting will feature a special presentation by our very own PNTC director Patricia Valverde on Ethical Issues and Considerations in PN CHW and PdS Work.

Dr. Valverde  is an instructor at the Colorado School of Public Health and the Director of the Patient Navigator Training Collaborative. She has extensive experience in research and program management in the areas of community outreach and patient navigation. She is a leader in local, national, and international patient navigation efforts.

PN CHW PdS Alliance Portal

The Housing Authority of the City and County of Denver (DHA) seeks a Patient Navigator to provide health care systems navigation for DHA residents seeking health care. Duties include designing and implementing health related programs, guiding residents through the health care system, helping residents to complete their insurance paperwork, assisting residents in obtaining financial help for their medical services, directing residents to their health care services for further diagnosis and treatment, identifying local resources and support, and keeping track of and helping residents get to their appointments.

This position pays $18.00/hr. with limited benefits.

Requires a four (4) year degree in social work, sociology, psychology, Human Services or related field, one (1) year experience in community involvement/organizing work, and experience working as a Community Health Worker or Patient Navigator.

Experience with low income, culturally diverse populations is required as well as experience in writing grant applications, grant monitoring, reporting and/or evaluation. A combination of appropriate education and experience may be substituted for the minimum educational requirement. A valid Colorado driver license and an acceptable driving record as outlined by DHA’s personnel policy are also required. Must have a working vehicle.

Native Patient Navigator Training: Communication and Outreach

The Native American Cancer Research Corporation is offering quarterly patient navigation training. The focus of the June training is on communication and outreach. This training focuses on motivational interviewing, team work and collaboration, confidentiality and privacy and professionalism. Each topic area will include participant activities or practical exercises.

The registration fee is $550 per participant. There are a limited travel scholarships for each training. Please request the partial scholarship when submitting the registration form. Youth Mental Health First Aid USA is an 8 hour public education program which introduces participants to the unique risk factors and warning signs of mental health problems in adolescents, builds understanding of the importance of early intervention, and teaches individuals how to help an adolescent in crisis or experiencing a mental health challenge.

Mental Health First Aid uses role-playing and simulations to demonstrate how to assess a mental health crisis; select interventions and provide initial help; and connect young people to professional, peer, social, and self-help care.

After attending this workshop, human services professionals will better understand the specific issues faced by adults who have grown up with drug and alcohol addiction and children still living in a home that is facing these issues. We will focus on how children are affected and different tools professionals can use to help these kids and their families. We will explore the healing journey for children and their families. Using the Betty Ford Center Model, participants will learn effective strategies to make a difference in the lives of children and adults; understand our unique prevention model; and explore new tools for working with children, parents and families.
