This course provides tools and strategies to help clients around diet, exercise, diabetes and cardiovascular disease prevention and control.

Advanced Health Behavior Change focuses on chronic disease, including diabetes and cardiovascular disease prevention and control. It is hybrid course which includes an online component and the one day skills practice workshop designed for patient navigators, care coordinators, clinical and non-clinical practice staff.

The in-person session provides for practice and observation of the behavioral skills.

The online portion includes pre-work required for the in-person workshop and disease modules that can be viewed at any time.

Then this course is for you! Learn more about defining the PN role, how to craft job descriptions, and  how to identify the training needs of your patient navigator staff.

The course provides a basic understanding of the patient navigator intervention model, reviews challenges facing PN program managers and provides strategies for an effective PN program.

We will review identifying evidenced-based clinical guidelines utilizing registries for identifying patients, developing standard work flows, PN training, key supervision needs and evaluation of PN services.

When: July 27th – 31st, 8:30 am to 4:30 pm daily (5 days)

Location: Regis University in Colorado Springs

The training workshop will:

  • expand the participant’s cultural diversity competencies
  • provide the knowledge/resources/practices necessary to enhance their communication efficacy with other professionals and community members
