You won’t want to miss the keynote speaker Wendy S. Harpham, a physician and 26-year cancer survivor who has used her unique perspective to raise awareness among clinicians and empower patients to talk with their healthcare team about the challenges of survivorship.

The event will have a Patient Navigation “track” that includes sessions geared toward PNs, including:

  • Patient Navigation Across the Continuum of Care
  • Compassion Fatigue
  • Patient Assistance Panel (to learn about pharma assistance opportunities and other patient assistance resources)
  • Rural and Frontier challenges, barriers, and successes
  • Motivational Interviewing Training

There will also be sessions geared toward patients and caregivers but could also be of interest to patient navigators and other health care workers, including:

  • Understanding risk factors and increasing healthy behaviors

The ability to understand, interpret, and process health information is known as health literacy. Patients need to understand health messages in order to make appropriate health decisions for themselves and family members, and patient navigators are essential in helping communicate this important information.

The Advanced Health Literacy course is designed to help Patient Navigators recognize patients with low health literacy and how to increase comprehension, readability, retention and apply skills to increase health outcomes.

Learning Objectives

This 1-day training will be highly interactive and activity based. By the end of the training, you will be able to:

  • Identify a variety of health education materials that are appropriate for your patients/clients.
  • Modify your verbal communication (e.g. word choice, explanations) to increase patient/client understanding of health-related information.
  • Use appropriate strategies to checking for understanding in order make sure that the patients/clients that you work understand important health information.

This workshop will be held in partnership with the Patient Navigator/ Community Health Worker/Promotor de Salud Alliance and will include a meeting over lunch featuring PN work and opportunity in Southern Colorado. The featured speaker will be Michelle East with Pueblo Community Health Center.

This training focuses on motivational interviewing, team work and collaboration, confidentiality and privacy and professionalism. Each topic area will include participant interactivities or practical exercises.

Registration Deadline: August 31, 2017

  • Training 1 – Day 1 and 2 – September 11-12: Cancer Content (13 hours and 30 minutes continuing education)
  • Training 2 – Days 3 and 4 – September 13-14: Patient Navigation Communication, Outreach, Literacy and Materials (13 hours and 30 minutes continuing education)
  • Training 3 – Day 5, September 15: Motivational Interviewing (6 hours and certificate upon completion)

This course will examine the effects of social determinants of health such as economic status, stigma, homophobia, racism and other social and political issues on risk, prevalence, and access to services.  A model and continuum of approaches will be introduced to assist prevention and care providers with implementation strategies to address these concerns.  Participants will become familiar with strategies for improving health equity so that individuals are more empowered to make healthy choices regardless of their living condition, income, sexual orientation or ethnic background.

 

This course is delivered by our partners, Denver Prevention Training Center.

She has extensive experience in research and program management in the areas of community outreach and patient navigation. She is a leader in local, national, and international patient navigation efforts.
