Responsibilities will include:

  • Building trusting, longitudinal relationships with our patients through in-home, virtual, and community-based visits
  • Collaborating with our clinicians by joining their medical visits and helping create a patient-centered care plan
  • Conducting an initial assessment for all patients to identify their goals and needs
  • Connecting patients to local resources and social services, for example helping them apply to SNAP benefits, accompanying them to a senior center, or researching community support groups
  • Supporting patients with chronic disease to take charge of their health, manage their medications, and stay out of the hospital
  • Helping patients navigate the healthcare system by accompanying them to doctor’s appointments, visiting them when they’re in the hospital, and helping coordinate their appointments
  • Providing health coaching to help patients with behavior change like exercise, smoking cessation, or healthy eating
  • Working with your team to build and keep up to date a resource directory of local and national resources (i.e., transportation, housing, financial, food, medication discounts, support groups)
  • Documenting all care and patient communication in our electronic medical record
  • Attend regular team huddles and case conferences

Required

  • Resident of the Denver Metro area with an understanding of local resources and experience working with patients to access them

  • You understand that the role is a hybrid role split between 2 days per week in home visits, 2 days per week in office & 1 day per week remote
  • Your attributable geography (subject to change) can cover an area that will require variable drive times but will fit visit and drive time within the work day
  • Must have a valid unrestricted driver’s license and access to an insured vehicle for daily use as driving is an essential function of the role

Preferred

  • Community Health Worker Certification or an Associates degree or higher in health sciences or related field
  • 3+ years’ work experience in CHW or case management role
  • Experience providing home visits and/or telehealth
  • Experience navigating hospitals and health insurances
  • Proficiency with electronic medical records or other documentation software

Salary: $26 – $28 an hour
The US base salary range for this full-time position is $26-$28/hour + equity + benefits. Our salary ranges are determined by role, level, and location. The range displayed on each job posting reflects the minimum and maximum target for new hire salaries for the position. Duties
The primary duties of the Health Navigation Specialist include, but are not limited to the following:

1. Community based health navigation work. These duties play a crucial role in assisting individuals in navigating the complex healthcare system and accessing the resources and services they need. The role includes providing support, information, and referrals to individuals seeking healthcare assistance. The primary focus will be on assisting community members in understanding their healthcare options, connecting them to appropriate services, attending medical appointments as appropriate, and empowering them to make informed decisions regarding their health and well-being.

Primary Responsibilities:
Client Assistance and Support
• Provide one-on-one assistance to individuals seeking healthcare services, including understanding insurance coverage, finding healthcare providers, and accessing necessary resources as needed. Duties may include recruitment of individuals who need this help from within the community.
• Conduct assessments of individuals’ needs and health-related concerns to identify appropriate services and support.
• Empower individuals to take an active role in managing their health by providing education, information, and resources.
Healthcare Navigation
• Assist individuals in understanding and navigating the healthcare system relative to their expressed and identified needs, including helping them schedule appointments, coordinating referrals, attending appointments, and explaining medical procedures and terminology.
• Collaborate with healthcare providers, community organizations, and social service agencies and referring agencies to facilitate access to healthcare services and address barriers to care.
• Stay up-to-date on available healthcare programs, insurance options, community resources and attend regular training as determined by the FRAHEC in order to provide accurate and relevant information to individuals.
Care Coordination and Referrals
• Coordinate care for individuals by ensuring timely communication between healthcare providers, community organizations, and other identified parties.
• Make appropriate referrals to community resources, social services, and support groups based on individuals’ needs and preferences.
• Advocate for individuals to ensure they receive comprehensive, coordinated, and culturally appropriate care.
Health Education and Promotion
• Conduct health education sessions and outreach activities as needed
Documentation and Reporting
• Maintain accurate and up-to-date records of client interactions, services provided, and outcomes achieved in accordance with agency policies and procedures.
• Compile data and prepare reports in accordance with agency policies and procedures including but not limited to staff meetings and meetings with agency staff as to be determined by the supervisor.
• Ensure and demonstrate compliance with privacy and confidentiality regulations

2. Peer-led trauma relief work. This work requires organizing groups of peers who are suffering from the impacts of trauma, and utilizing the TREE model with them. This will entail the following:
• Learning the TREE model,
• Working through the TREE model with fidelity with the participants,
• Interacting with any researchers working on the project, and
• Collecting and providing data when necessary.

3. Social Determinants of Health. This may include, but is not limited to, helping community members address barriers regarding public benefits, housing, employment, schooling, etc., as relates to their overall health and well-being.

Compensation and Benefits
• Salary Range: $43,000 – $47,000 annually
• SEP IRA retirement contribution equal to 10% of salary
• QSEHRA health reimbursement up to $400 per month
• Flexible time off policy

This individual acts as a single point-of-contact to coordinate resources along the care delivery spectrum, identify gaps, and provide proactive follow-up. The Care Coordinator is responsible for making sure the patient’s care at various locations is connected and there are no gaps in care or communication. This role will report to the Lead Care Coordinator.

The Day to Day

  • Performs outbound calls to patients to understand their clinical needs and connect them with appropriate resources. Performs outbound calls to providers to make appointments for patients or follow up on care. Answers inbound calls from patients, providers, and other resources.
  • Follows up with patients to ensure their needs are met and schedules future check-ins. Notifies patients of location and appointment times as needed.
  • Coordinates with clinical resources and providers to ensure smooth continuum of care for patients. Assists with completing applications for resources, paperwork for provider visits, etc.
  • Monitors patient hospitalizations and follows up as necessary with care team members and outside resources to confirm Strive gathers all relevant patient information.
  • Provides patients with education materials and sends communications to primary care physicians, nephrologists, and specialists for new enrollments/appointments.
  • Collaborates well with all levels of a clinical team (from Medical Assistants to Physicians) and partners closely with the Strive Nurse Practitioner (NP) to manage all pieces of care related to resources, appointments, care transitions, and care gaps.

Minimum Qualifications

  • 2+ years combined of related education, experience, or certification.
  • Current BLS or CPR Certification required.
  • Efficient and reliable transportation allowing for the ability to commute to patient homes.
  • Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting.
  • Internet Connectivity – Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms.

Preferred Qualifications

  • Active Community Health Worker (CHW) Certification.
  • Customer service experience.
  • Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook.

Jared Polis signs a bill increasing access to remote patient monitoring.

Colorado Gov. Jared Polis signed legislation on May 29, 2024 to increase access to telehealth services in rural and underserved areas, and to better combat cardiac disease.

The law expands access to Remote Patient Monitoring (RPM) services for Medicaid members across Colorado and provides resources for healthcare practices serving rural underserved populations to obtain RPM equipment. The law also expands access to Continuous Glucose Monitoring (CGM) devices.

“Coloradans in my district know firsthand how challenging and expensive it can be to access critical health care services,” said Senator Dylan Roberts, D-Frisco, in a press release. “During my time in the legislature, I’ve worked hard to deliver high-quality, affordable health care services to underserved and rural areas and this law expands access to remote health services and continuous glucose monitors, which have proven to be successful at ensuring patients receive the right care, at the right place, at the right time and save so much money in the long-run.”

Remote patient monitoring (RPM) is a type of telehealth that enables healthcare providers to monitor patients outside the traditional care setting using digital medical devices. Examples of devices include weight scales, blood pressure monitors, pulse oximeters, and blood glucose meters. RPM connects patients to their local provider via telehealth, leveraging technology while maintaining continuity of care.

Reviewer expectations:

    • Become familiar with the Training Program Standards (the final version will be shared).

    • Review each application and additional documentation:

      • CDPHE will convene a Review Committee to review each application and will provide feedback to the applicant within 30 days of submitting.

    • Provide feedback using a checklist or rubric.

  • CDPHE anticipates about 5 applications with an additional 1-2 in next year.

Values are our principles—like being there for family, or wanting to be healthy. You can help your clients think about the values that guide their lives. In this workshop, you will examine your own values as well as learn techniques for using values in your discussions with clients.

The sessions will be offered virtually and in-person on the Anschutz Medical Campus in Aurora.

  • May 18 (virtually)
  • June 4 (in-person)
  • June 6 (virtually)
  • June 11 (in-person)
  • June 14 (virtually)

To register for the assessment, individuals must have completed verified coursework from a state-recognized training program (Otero College, Patient Navigation and Community Health Worker Training program, Metro State University at Denver or the CU Multidisciplinary Center on Aging). Please complete the Colorado Health Navigator Registry Application to verify training prior to registration for the assessment. After completed training is verified, you will receive a link to register for the assessment. Registration must be completed 30 days prior to the date of the assessment.

As a reminder, the required coursework to qualify for the assessment under under PNCT is:

  • Level 1 Health Navigation Fundamentals
  • Basic Motivational Interviewing (if Level 1 taken online)
  • Level 2 Care Coordination

Medicare will now provide reimbursement for select patient navigation services. This is an exciting policy update that will provide sustainable funding to those who are helping patients navigate complex care and chronic conditions.

Under the 2024 Medicare Physician Fee Schedule, the Centers for Medicare & Medicaid Services (CMS) created four CPT codes that will allow health care providers to bill for services.

Specifically, the PFS included new billing codes for:

  • Social Determinants of Health (SDOH) risk assessments: SDOH risk assessment refers to a review of an individual’s SDOH or social risk factors that influence diagnosis or treatment of medical conditions.
  • Community Health Integration (CHI) services: services performed by certified or trained auxiliary personnel, including a community health worker, under the direction of a physician or other practitioner, to address SDOH needs that affect diagnosis and treatment of a patient’s medical problems.
  • Principal Illness Navigation (PIN) services: services performed by certified or trained auxiliary personnel, including a patient navigator or certified peer specialist, under the direction of a physician or other practitioner, to help patients diagnosed with a serious, high-risk condition.

The finalized CMS rule stipulates that navigators must be credentialed to participate in the CMS reimbursement program. In states that have a certification, navigators must hold this credential to receive reimbursement. In states that do not have applicable licensure, certification or other laws, CMS requires personnel be trained to provide these services.

Colorado has a state-recognized Health Navigation assessment and registry (soon to be the Community Health Worker credential); navigators and CHWs who are listed on this registry are eligible for reimbursement under this new Medicare rule as well as new Medicaid rules (to be implemented in 2025).

CMS-required training competencies include:

  • Patient and family communication
  • Interpersonal and relationship-building
  • Patient and family capacity building
  • Service coordination and systems navigation
  • Patient advocacy
  • Facilitation
  • Individual and community assessment
  • Professionalism and ethical conduct
  • Development of an appropriate knowledge base, including specific certification or training on the serious, high-risk condition/illness/disease addressed in the initiating visit.

For those seeking training, the Patient Navigation and Community Health Worker Training Program (PNCT) can provide courses on these topics. Training is available in-person (Denver area) or online. The courses are led by a team of expert instructors, with skill-based, interactive activities.

The Colorado Department of Public Health and Environment (CDPHE) would like to introduce a Community of Practice for Community Health Workers. If you are a CHW, Health Navigator, Patient Navigator, Promotora de salud or similar role you are invited to join a network of CHWs from all over Colorado, to share best practices and identify training opportunities that fits your needs. The Community of Practice will be designed for Community Health Workers from all over Colorado who are working in health or community-based settings to learn from one another, share resources and best practices and provide you the opportunity to explore new training opportunities in your roles.

In addition, CDPHE is providing the opportunity to learn more about diabetes and cardiovascular prevention and management and how you can incorporate programs such as National DPP and DSMES into your communities.

PNCT Presents CHW Work at Three Fall Conferences

One poster and three academic conferences later – our poster has been to places. Attending different research conferences gave me unique insights into the diverse landscape of academic gatherings. I wanted to share the highlights of my academic adventure and the valuable lessons I learned along the way. But before diving into that – let me share a bit about the poster I presented.

The title of our poster is “Building Workforce Pathways for Community Health Workers in Colorado.” This work was with the Patient Navigation and Community Health Worker Training program (PNCT). PNCT is passionate about bridging the needs of formalizing pathways and creating sustainable career ladders for community health workers (CHWs). This poster was to emphasize our current CHW training program, which is funded by the Health Resources and Services Administration (HRSA). This program aims to establish a strong foundation and establish infrastructure for education and career pathways for aspiring CHWs. The program offers training that aligns with CHW competencies, apprenticeships, and internship opportunities to prepare individuals before employment.

Conference 1: Public Health in the Rockies (Keystone, Colorado)

This was my first conference as a student and presenter in Keystone, Colorado – a beautiful and serene location. This conference may have been smaller and more intimate than others I’ve attended, but the quality of work was exceptional. Many people showed interest in my poster by asking thoughtful questions and commenting on our significant work. Since moving to Colorado and attending this conference, I have gained a greater understanding of the incredible local work and the powerful collaborations happening beyond state lines.

Conference 2: Colorado School of Public Health (CSPH) Research Exchange (Aurora, Colorado)

Our poster made its way to the CSPH Research Exchange where “faculty and students from all three of our universities—University of Colorado Anschutz, Colorado State University, and University of Northern Colorado—and community partners who share our dedication to public health are invited to participate.” I was eager to showcase the work we’ve been doing with CHWs and be surrounded by peers and colleagues to learn more about their current work. The Research Exchange program featured presentations and sessions from experts in different fields, making it a multidisciplinary learning experience.

Conference 3: American Public Health Association (Atlanta, Georgia)

The last poster stop – Atlanta. This was a great way to end the year and also end the run for my poster adventure. This year’s theme was “Creating the Healthiest Nation: Overcoming Social and Ethical Challenges.” CHWs play an immense role in overcoming barriers like this. Although I learned about different efforts around CHWs, they had a common goal of empowering CHWs and overall improving the communities they serve. I was grateful to travel out of state to share impactful local work from Colorado.

I am excited to see what the future holds with PNCT, the team, and our work with CHWs. If I could sum up this whole experience in three words, it would be: enlightening, inspiring, and memorable.

Language Survey
We are conducting a survey to assess the language needs of diverse community health workers and patient navigators who serve multi-lingual community members and individuals in Colorado. PNCT is requesting your input on a brief survey that should take no more than 5 minutes. In 2024, we have funds to offer our trainings in a third language (other than English and Spanish). Your response will help us determine what languages navigators and CHWs would prefer to learn in.

Colorado Gov. Polis signs the CHW bill.

SB23-002, Medicaid Reimbursement For Community Health Services, was signed into law by Governor Polis on May 10.

SB23-002 sets up a process to enable some of the services provided by Community Health Workers, like health education, health navigation, and referral to community-based resources, to be reimbursed by Medicaid.

Colorado’s Health Care Policy and Finance (HCPF), the state Medicaid department, will conduct a stakeholder process to finalize what will be included in the State Plan Amendment (SPA), which is the federal authority needed from the Centers for Medicare and Medicaid for state Medicaid to start reimbursement of these important services. Understanding Microaggressions: Why They Are Not Micro At All

What are “microaggressions” and how do they impact health equity? Microaggressions, or subtle acts of exclusion, are rooted in implicit bias and are more than just insults or insensitive comments. These everyday slights have a harmful cumulative effect. But you can make a difference! In this 2-hour workshop, you will learn how to examine your own perceptions, as well as what to do if you commit, experience or witness a microaggression. This training will feature small group discussion using breakout rooms.

Digital Readiness and Building Digital Health Literacy

In this 1-hour interactive webinar, the American Health Information Management Association (AHIMA) Foundation will provide an overview of digital health literacy and digital readiness in combating health inequities. Disability and Ableism 101

Over 20% of the U.S. population identifies has having one or more disabilities or impairments according to the 2010 U.S. Census, yet people with disabilities and those who identify as disabled are often left out of conversations around diversity and inclusion. This workshop will cover some of the history of language used by and about this community, what ableism is (as well as able-bodied/neurotpyical privilege), ways to dismantle these types of oppression so ingrained in various systems, and ways that to create more inclusive programs, conferences, classrooms, offices, and health care to ensure that all members of this community are able to engage.

Andrea Dwyer, University of Colorado Cancer Center, Colorado School of Public Health, traveled to Washington, DC, to participate in the White House Cancer Moonshot Colorectal Cancer Forum on March 10. Dwyer directors the statewide Colorado Cancer Screening Program.

In recognition of March as Colorectal Cancer Awareness month, the White House Cancer Moonshot convened leaders from the colorectal cancer community, including patients, survivors, private sector companies, foundations, academic institutions, and health care providers focused on accelerating efforts to reduce the deadly impact of colorectal cancer.

“I was excited to join Swati Patel, MD, my Colorado colleague, and about 50 others to join in the Colorectal Cancer Forum at the White House on March 10. 2023.  It is my hope that the Biden team and all of our elected officials will truly take this opportunity to embrace the power of cancer early detection, outreach and seize opportunities to develop new and cutting edge treatments.  The brain trust, passion and energy at the White House is what it will take to achieve a Moonshot, the time is now.
