Module 2: Components of patient-centered models of care
What is patient-centered healthcare? Healthcare for the patient.
Patient-centered health care is a partnership between the healthcare team, patients and their families. Patient-centered care respects patients’ values, preferences and needs. It provides patients with education and support so they can make informed decisions and fully participate in their own care.
Patient-centered care may also include access to emotional support, continuity between health care settings, physical comfort, and coordination of care.
Are physicians patient-centered? Not as much as they would like
Results from a national survey of primary care physicians showed that:
Physicians favor the idea of patient-centered care:
- 87% support an integrated, team-based approach to care
- 83% believe that patients should have easy access to their medical records
But they have not yet integrated patient-centered care into their practice:
- 74% report problems getting medical records or test results for patient visits
- Only 48% provide patients with reminders for preventive or follow-up care
- Only 22% scored high for implementing patient-centered care
- Only 36% ask patients for feedback
