Module 2: Components of patient-centered models of care
Module one overviewed patient navigation and its role in improving patient care and reducing healthcare disparities. In this module we will see how provider-centered care—versus care that is centered around the patient—can create barriers. Also, since patient navigators work with providers to reduce barriers created by the healthcare system, we will look at healthcare team member roles and patient rights and responsibilities.
