Overview

This course is organized into three modules that describe patient navigation’s role in reducing health disparities and improving treatment for chronic diseases. The three modules cover:

Module 1: Patient navigation and health disparities

• Definitions and models of patient navigation

Module 2: Components of patient-centered models of care

• Patient-centered and provider-centered care
• Roles of healthcare team members
• Patient rights and responsibilities

Module 3: Patient navigator roles & responsibilities

• Patient navigator qualifications and characteristics
• Developing effective client relationships
• Staying within personal and professional boundaries
